Increase your revenue by implementing Chronic Care Management Services
The TMF Quality Innovation Network Quality Improvement Organization is offering free consulting services and technical assistance to primary care physicians interested in implementing Chronic Care Management services for their Medicare patients. The CCM billing codes allow physicians to be paid for non-face-to-face care coordination services provided to Medicare beneficiaries who have two or more chronic conditions. For most physicians, CCM is a way to be paid for care they are already providing. The TMF QIN-QIO is recruiting physicians until Dec. 31, 2016.
